Summer Kempton Soccer Scholarship

Folsom Athletic Association Applicant Instructions

This scholarship is for Soccer only.

Please read the following instructions completely before filling out your application.

1- The applicant must be a Folsom resident to apply for an FAA Scholarship.

2- Attach a copy of a recent Folsom Utility Bill or other proof of residency to the application.

3- The application must include a physical Folsom address. P.O. Box will not be accepted as a valid Folsom address.

4- Applicants must be 17 years or younger.

5- When completing the application, DO NOT leave blanks. Use DNA (Does Not Apply) or NONE to complete blank responses.

6- Include as much information as possible in order to evaluate your application.

7- Once you have completed your application, return the application to your Youth Sports Administrator. DO NOT send applications to Folsom Athletic Association.

8- Your Youth Sports Administrator will notify you if your application is accepted for assistance.
